If you are pregnant, planning a preganancy, or have questions regarding breast feeding and an exposure to a medication, drug, infection, or a chemical, the staff of the Illinois Teratogen Information Service (ITIS) are trained to answer your questions.
You may contact ITIS directly at (800)252-4847 or (312)981-4354 with your inquiry. Or you may complete the following FORM. All information is kept strictly confidential.
This form can be emailed directly to ITIS, or you can printed out and faxed to ITIS at (312)981-4366 along with any additional supporting information you feel is relevant.
After ITIS receives the form, you will receive a phone call from a teratogen information specialist to inform you of the current information regarding your exposure.
You will NOT be contacted via e-mail.
After the phone conversation, you will be given the option of receiving a letter summarizing the information.
