FOR REFERRING PHYSICIANS
If you are a physician with a pregnant patient who
has been exposed to a drug, chemical, virus, or other
environmental agent that you think may put the fetus in danger of
a birth defect, we can provide you with:
- An assessment of the risk
- Supporting documentation.
You can get this information in one of three ways:
- Call the Illinois Teratogen Information Service (ITIS) at (312)981-4354
or (800)252-4847; calls will be returned Monday thru Friday between 9 a.m.
- 4:30 p.m. (CST).
- Complete the form that is linked to this page by clicking here or below. This form asks
you all the questions we would ask you in a typical teratogen intake procedure.
We will do the research and provide you a risk assessment within 1-2 days
by email or contact you by phone for further discussion. If the intake is
not complete or we have additional questions, we will contact you by telephone.
Please provide as much infomation as possible.
